Acne develops when the hair follicles - the pores in the skin - become clogged with dead skin cells and/or sebum. In addition, the connective tissue layer is often affected, where inflammatory nodules form.

Acne typically develops on the face, neck, chest, back and shoulders, as these areas of the skin have the most sebaceous glands.

What is the reason for its development?

Each hair follicle has a sebaceous gland in it. The secretion produced by the sebaceous gland (the sebum) comes out through the hair follicle to the surface of the skin and forms a protective coating on both the skin and the scalp. If the body produces increased amounts of sebum or if the epithelial cells are destroyed and block the pores, the sebum accumulates in the hair follicle and solidifies there, forming a plug.

Usually other pores in the skin, such as the sweat glands, are not involved in the development of acne.

The most common causes of skin imbalances:

- overproduction of sebum - e.g. due to hormones, stress, chemicals, steroids, Demodex mites

- irritation of the skin's hair follicles due to inadequate exfoliation of dead skin cells

- the increased destruction of skin cells leads to blockage of the skin's pores and sebum ducts 

- microbiological imbalance is disrupted and pathogenic agents appear on the skin and in the skin's internal structure (overgrowth of Demodex mites, bacteria such as Propionibacterium, Bacterium oleracea, etc.)

How can you apply?

- Closed comedos - The opening of the hair follicle is blocked by dead skin cells and sebum, causing the hair follicle to dilate and the normally produced secretions to accumulate.

- Mites (open comedones) - Similar to closed comedones, but darkened and open towards the surface of the skin so that their contents can be drained.

- Pimples - Pimples are reddish patches of skin that protrude from the surface of the skin and are caused by inflammation and/or infection of blocked hair follicles. When healing, the acne tissue damage acts like a small wound. The scar tissue that closes the wound, which is initially normally red and protrudes slightly from the skin, later fades. 

- Cysts (hose) - Cysts are deeper connective tissue nodules that protrude beneath the surface of the skin and are filled with accumulated mucus due to a blockage in the hair follicle. They often heal with scarring. Keloid scarring acne protrudes from the skin surface and has a familial appearance. Scars with tissue loss are the more common, healing and indenting into the skin surface in a similar way to scars from chickenpox. These are known as ice pick scars. The scars can be excellently treated with crystalline microdermabrasion. 

Usually the cause and the forms are mixed and appear on the face. Accordingly, a distinction is made between types and care methods.

Dilated pores, mites and skin comedones can themselves maintain the shift in skin balance. It is mainly the closed dermal comedones (demodex nodules) that can trigger an ongoing immune reaction and inflammation in the body as a chronic nodule. If the skin is stressed for a long time, the development of acne rosacea is common.

Who is more prone to acne?

Hormonal activity can predispose you to acne or make existing symptoms worse. It can occur in teenage boys and girls; in girls and adult women in the few days before menstruation; in pregnant women; during menopause and when taking certain medicines such as cortisone.

Recent research has found a link between hormonal changes and the development of Demodex mite infections: changes in hormone levels alter the composition of skin sebum in a way that favours Demodex mite reproduction. This microscopic mite lives in the sebaceous glands of the facial skin, feeding on the protein and fat found there.

In a scientific step forward, a survey of clinical trials published between 1950 and 2011 (63) on acne and Demodex brevis mite infection was carried out in 2011. The duration of acne skin resurfacing depends largely on how long the process takes and how manageable and sensitive the skin is.
